After School Registration Form

In order to retain my child's spot in the program I am required to pay weekly the full tuition regardless of their attendance, breaks, holidays, inclement weather school closings or any school closures. I still have to be when school is on break during fall break, winter break and spring break. The rate is $40 for non-attendance tuition on those weeks and the camp rate for those attending.

I will do my best to encourage my child to follow the program's Code Of Conduct 1. Respect 2. Participate 3. Have a positive attitude. If my child fails to follow the code they will be given 1. A VERBAL warning 2. A WRITTEN warning 3. A 2-3 Day paid suspension 4. Expulsion from the program. This applies to parents as well. We want everyone to feel good at the program and have open communication

I understand my child has to be picked up before 6pm. Any minute after is charged at $1/min. Additionally should I want to withdraw or remove my child from the program I need to give 1 week's notice on Friday prior to the new week starting.

I understand my tuition is due on Fridays before midnight for the upcoming week. Anything past that is late and will have a $10 late fee added. The latest you can pay is Monday with the $10 late fee. Anything past Monday considers your enrollment void and you will need to re-enroll.
